School Entry Immunisation Certificates
You will need an Immunisation Certificate to enrol your child in primary school. This certificate gives us a clear record of your child’s immune status and helps the school to do the right thing by each pupil should a case of infectious disease be discovered in the class.
Some children have health conditions that make immunisation unwise. And some parents have conscientious objections to immunisation. These will be recorded on the certificate and the child will be accepted for school. The child’s immunisation status is then on record and can be vital in quick action to protect them.
How To Obtain a School Entry Immunisation Certificate in the City of Kingston
This certificate is provided by the Immunisation team and the Maternal and Child Health nurses only. This may change to include doctors in the future.
To obtain a certificate you have several options:
1. Take the Child Health Record Book with the immunisation history to any Customer Service Centre in the City of Kingston. Details will be photocopied and if the record is up-to-date, a certificate will be posted to the parent/guardian.
2. If the child needs further immunisation, the doctor may be visited, or the Council immunisation session where the immunisation and the certificate can be received.
3. To obtain the certificate from the Maternal and Child Health nurse, please ring for an appointment, and take your Child Health Record Book with you.
4. If a child cannot be immunised for medical reasons, obtain a medical declaration form from Customer Service or Immunisation team. Return it after your doctor has completed it. An incomplete School Entry Immunisation Certificate will be sent to you.
5. If parents have a conscientious objection to immunisation or the child’s records have been lost, a statutory declaration form needs to be completed. These are also available at Customer Service Centres or the Immunisation nurse. The appropriate School Entry Immunisation Certificate will be sent to you.
